As website owners who have unfortunately experienced the transitioning of both parents we recognized the importance of creating a website that would help people gather and track the essential documents one needs to manage the multiple classes of assets involved (real estate, personal property and insurance etc.),  when a person transitions from life.

The following list is what we have found to be the key forms and primary information one should start with, when discussing and acquiring the asset information from your elder or for yourself. Should there be an attorney, accountant, financial advisor and tax advisor or other professional who has worked with the individual in the past, please contact, and consult with them as well. The information you acquire is necessary to fill out the forms provided on this website when you begin this essential documentation process.

          Other considerations involved in this challenging period are the well-meaning but unauthorized  relatives, friends, etcetera, who may try to take advantage or simply interfere at this traumatic and challenging time period in the person's life. Our suggestion is to ignore these major distractions and their efforts to try to capitalize on the challenging situation.

FORMALLY establishing who has final authority to decide issues of care, and postmortem care, is the most crucial information and important documentation necessary to fulfill the wishes of the person who is transitioning. Assuring these tasks are completed goes far towards making sure you are following the wishes of the person you love so dearly.
